Low Lovely Smile CS face accessory does nothing

it looks like its working tbh, its just applying a VERY faint blush over the cheeks. i can't find any image online of what it's supposed to look like, and it's not available in the offline version of BannedStory I have either..

it definitely shows up in cash shop though and in your example pictures you can see it as well (maybe your monitor brightness/contrast is too high so it covers the effect? it is VERY faint.)

 

I can confirm the Lovely Smile face is working correctly (including on a character with your same face but sapphire). It adds a faint blush to the cheeks and similar color eye shadow above/behind the eyes. Your character's face already comes with a faint blush and your NX eye accessory covers most of the eye shadow so it's not very noticeable on your character. Also you can't really use the item art to judge what something is going to look like in this game. A lot of items have fairly different item art compared to what it looks like equipped.
